We can help you understand and fill out a healthcare proxy form, living will, or another advance directive. We can also notarize these documents.
Anyone can contact an MSK patient representative.
It’s OK if you don’t want to give your name or other details. However, it makes it harder to help you if we don’t have specific information about you. No one at MSK will treat you differently because you contacted a patient representative.
